Billing at Eye Associates Northwest
Eye Associates Northwest is a participating provider with most major medical and vision plans including Medicare and Medicaid.
It is our recommendation that you contact your insurance carrier prior to making an appointment at our office to determine your benefits, eligibility and our participation. We have limited knowledge of your insurance plan and any information we provide is not a guarantee of benefits, eligibility or participation.
Please make arrangements for your referral, if needed, prior to your appointment.
It is advised that prior to scheduling an appointment for a vision exam, you contact your insurance to verify your eligibility. Vision exams are allowed once every 12 or 24 months depending on your insurance plan. Vision exams scheduled too soon will not be covered by your insurance and will become your full responsibility to pay.
Co-pays are due at the time of service. We accept cash, checks, Visa, MasterCard, American Express, and Discover.
Patients without insurance coverage will receive a 15% discount on services paid the same day (this discount does not apply to co-pays, facility fees, glasses, contact lenses, medications or other products and supplies and savings will be based upon the current Eye Associates Northwest fee schedule).
